St. John Providence Health System would like to congratulate all our health care providers, and the staff they work with, who have recently been designated by Blue Cross Blue Shield of Michigan as a Patient-Centered Medical Home. These doctors offer their patients a team-based model of care that's led by a personal physician who provides continuous and coordinated care throughout the patient's life to help maximize healthy outcomes. It's just the kind of forward thinking that you'd expect from the caregivers at St. John Providence Health System who have always had a passion for putting the patient first and providing coordinated care for the mind, body and spirit. 

The Patient-Centered Medical Home model represents a change in the way a primary care physician approaches patient care, thereby strengthening the doctor-patient relationship. In the PCMH model of care, a primary care physician tracks and monitors the patient’s overall health. He or she does this by teaming up with other health care professionals in the practice and beyond to make sure that all the patient’s health care needs are being met — from diet and nutrition to testing and monitoring. The primary care physician tracks the patient’s progress, and the team guides the patient to a healthier lifestyle that should reduce the need for medical care in the future.
