If you are age “55 or better,” please fill out the CareLink enrollment form or call St. John Providence SeniorLink at 1-888-751-5465 (1-888-SJ1-LINK) for a temporary membership card and CareLink brochure with membership application. Within six weeks of mailing in your application, you will receive a CareLink welcome mailing.  This mailing will contain your official CareLink membership card and membership details.
