About Pre-Registration
For your convenience, this site can be used to pre-register prior to your scheduled procedure. Completing and sending this form will decrease your wait time the day of your visit by allowing us to have most of your paperwork ready for you when you arrive. Pre-registering also allows us to verify your insurance benefits, ensure that your procedure has been authorized and notify you of any co-pays or deductibles that may be due.
All information submitted in the online pre-registration will be treated as a confidential part of your medical record. The information is only available to staff for the purpose of pre-registering you and verifying your insurance coverage.
By submitting this pre-registration form, you are authorizing St. John Health to contact your insurance company for verification of coverage and authorization for services.
Please note, upon arrival the day of your appointment:
- You will complete the registration process by presenting copies of your driver’s license and all insurance cards
- You will review the registration form for accuracy and sign authorization forms
- Medicare recipients will need to complete a Medicare Questionnaire
