Home evaluations are offered as appropriate. These address concerns you may have about moving around at home safely, medical equipment and home modifications.

Continuum of Care

We are committed to a continuum of care that begins with your entry into inpatient hospitalization, through your rehabilitation stay and continues after discharge to an outpatient treatment or home care program. St. John Macomb Hospital offers outpatient treatment at the hospital and in five outpatient facilities conveniently located in Warren, Clinton Township, Shelby Township and Rochester Hills.

Medical Equipment

Rehabilitation team staff members will assess your need for special medical equipment and either help you obtain the equipment or arrange for delivery before your discharge.
