Directory of Community Health Centers
If you're one of the many people in our community who are uninsured or underinsured, you'll be happy to know we're here for you. St. John Health Community Health Centers provide a wide range of primary care services, regardless of your ability to pay.
St John Providence Health System either directly provides primary care or partners with others in the community to provide health care to all those in need. Some sites charge a set fee or use a sliding scale to determine fees.
Services may vary dependent upon the site. These sites do not provide emergency care although patients may walk-in to certain sites and if time permits they may be seen.
Primary care services usually include the regular services you would receive from a family doctor. They might include preventive care, health education, treatment of long term (chronic) diseases like high blood pressure or diabetes. They also include care for acute (sudden) problems like cold, flu, gastritis, and bronchitis. Most of the listed heath centers also provide the following:
- Blood tests
- Preventive care and health education
- Management of acute diseases (like flu or strep throat) and chronic diseases (like diabetes, high blood pressure and asthma)
- Social services
- Pelvic exams and pap tests
- Pregnancy testing
Emergency services are not provided.
